95 Nissan Pathfinder- Engine bucking/jolting

It's a 95 Nissan Pathfinder, factory everything still in it. I drive it to school and work and whenever it's running, doesn't matter if it's moving or not, it physically jolts around every 3 seconds. Some days it is more intense than usual, but it's always there. I think it's gotten worse over the past month or two.
Any ideas?

Check the tune up items and vacuum leaks. any codes?
