Engine revs up and down in neutral Sign In

I have a '95 Isuzu pickup 2.3L manual 5sp tranny

The engine revs up and down when the rpms drop down to idle speeds, ie. nuetral. I can be coming to a stop sign throw it into neutral and it'll start reving up and down once again. When it first started my instinct were the vacuum hoses. They are old and needed replacing and in fact the occassional would fall off thus i'd put it back on and the problem would be fixed. Well I've since replace all the hoses with new hoses. I don't have scimatics for their exact location so i went out to a local junk yard and simply compared it to one they had out there. May not have been the best way but better than guessing as to where they're suppose to go. For a time it seemed to help or the revs would come back on occassions but now their back all the time. I've check to make sure none had fallen off but they all seem to be stable. What's the possibility of an O2 sensor? I'm sure it's the original one in and the truck is over 10 years old. Besides that nearly the entire exhuast system needs to be replaced probably including the manifold. I'm just waiting on my new flowmaster muffler to come in to have everything replaced form the catylitic converter back. I've tried readjusting the idler but if it's set to a rather high rpm then theres no problem. Again which sounds like a vacuum hose. Who knows maybe they're just placed wrong. In any case any other suggestions would help.
