Fighting for Air?

I have a 1999 Ford F150 pickup. This morning when I got in to start it up I noticed it "fighting for air". It seemed that the RPM was "jumping" from 1 to 3. Kind of making a roaring sound. It has 57,000 miles on the truck. Could this be a fuel filter or air filter issue? I recently purchased this truck for my son.

At the moment no clue so please say which engine and if it is cranking and not starting or starting and making this noise,

First eliminate all vacuum or air leaks in the system. You may have a bad IAC ( idle air control) but try cleaning the throttle body first. Remove the intake snorkel, have someone hold the throttle wide open for you and scrub the back side of the throttle plate and surrounding bore with an old tooth brush and some carb cleaner. Be sure to spray some into the small holes next to the throttle plate. That should help stabilize the idle. If it still has a problem, replace the IAC.

It is a 4.2L V-6 manual transmission. It starts fine. Does not die out. just sounds like it is fighting to stay running.

Is air intake all together? Clean and or replace IAC as HT indicated making sure everything is attached as designed for air intake. Could be some pretty wild vacuum leak but begin with that much.
It should be throwing a code but with the noise it's probably something more obvious. 1-3 RPM meaning 1,000 to 3,000 doesn't sound like it's about to stall but rather it's racing too high,
