There doesn't have to be water (I assume you mean coolant) coming out of the exhaust for it to be a head gasket. The coolant would more likely be burned off before it exits anyways. But, just because it's overheating doesn't mean it's a head gasket either. It could be as simple as a stuck thermostat. How's your coolant level? What you need to do is pressure test the cooling system and check for leaks.

There are a handful of professional mechanics on this site that will be able to give you much better advice than I can so be sure to check back.

Have you checked the water pump to be sure its still working properly? If you have a lot of blowby, have you checked or replaced the PCV valve? Have you done a compression test?

Rough idle could be from alot of things as your car is a 86.
But i would start with the idle air control vale, clean the Throttle body and clean the egr vale.(May need new gasket)
Does your car have engine light on?
You say you replaced wires, plugs and dist cap is that it or was it a full tune up including dist rotor, air filter, fuel filter, PCV, Thermostat and so on?
Does your car run on OBD2 emissions system or older charcoal canister type.
Purge vale for evap system can cause issues if not working.

Ron, really? Honestly if you have to ask if he has OBDII on a vehicle that was made a full decade before OBDII came out you should stick to the welding and not the diagnostics. We welcome people to give help to others here, but it needs to be help based on knowledge, facts and evidence not just throwing out random names of stuff and guesswork. No EVAP system is going to create blowby. No EVAP system is going to create idle problems.

Please limit your advice to things you have had firsthand experience with to give advice on.
